Instructions for Mentor Survey
If you only have one student extern, please follow the instructions on this page and complete the survey using the link at the bottom of the page.
If you have multiple student externs to evaluate, please click HERE.
1. The survey has 9 pages.
2. The mentor can complete the survey either:
a. one page at a time (returning to the survey periodically until completed)
b. or in one sitting
3. To advance the survey to the next page, click “Next.”
4. To return to the unfinished survey at a later date, the mentor must first complete the currently displayed page and then click “Save” at the bottom of the page.
5. The mentor can then close the web browser and return to the URL below (in number 7) to start the process again at the next page of the survey until the survey is complete.
6. Please note:
a. To resume the survey where left off, the mentor must use the same computer he / she started with; and
b. If the survey sits idle without the user clicking on “Save” the results of the survey may be disrupted and the mentor may be unable to complete the survey.
